Billing Policy

Financial and Practice Policy Agreement

Please read carefully before booking services with ONE Nutrition.

Insurance Billing

ONE Nutrition is in-network with many major insurance plans including Aetna, Blue Cross Blue Shield, Cigna, and others. We will submit claims on your behalf.

You are responsible for providing accurate and complete insurance information at intake. Benefit verification and any website estimate are not a guarantee of coverage or payment.

Patient Financial Responsibility

You are responsible for any portion of fees not covered by your insurance, including copays, coinsurance, deductibles, non-covered services, and charges resulting from a denied claim. This remains true even if benefits were checked before the appointment.

We will make reasonable efforts to verify your benefits prior to your first appointment. You are responsible for understanding your plan and for promptly notifying us of changes to your coverage.

Self-Pay Rates

Our transparent self-pay rates are $150 for a 90-minute initial visit and $135 for a 60-minute follow-up visit. Payment is due at the time of service unless we agree otherwise in writing.

Insurance estimates are not final bills. If your plan does not cover or denies a claim, you are responsible for the applicable agreed fee and any patient cost share.

Advance Beneficiary Notice for Medicare Clients

If you are a Medicare beneficiary, and if we believe Medicare may not cover a specific service, we are required to provide you with an Advance Beneficiary Notice prior to that service.

You will receive and sign a separate Advance Beneficiary Notice in your client portal for any applicable services. This is required by CMS and cannot be completed via website agreement alone.

Cancellation and No-Show Policy

We require at least 24 hours notice to cancel or reschedule an appointment.

Cancellations with less than 24 hours notice, or failure to attend without notice, are subject to a $75 late-cancellation or no-show fee. This fee is the client's responsibility and is not billable to insurance.

Repeated no-shows may result in discharge from the practice.

Late Arrivals

If you are more than 15 minutes late to your scheduled appointment, we may need to reschedule. The session will still be counted toward the cancellation policy if we are unable to provide the full service.

Payment Methods

A credit card or HSA/FSA card is required to hold an appointment. By providing a payment method, you authorize ONE Nutrition to collect any applicable patient cost share and any valid late-cancellation or no-show fee under this policy.

We accept major credit cards, debit cards, HSA/FSA cards, and other methods as displayed at checkout. Payment for self-pay services is collected at time of booking or at time of service.

Billing Disputes

If you have a concern about a charge, please contact us within 30 days of the statement date at info@optimalnutritionexperts.com or via our contact form at onenutrition.us/contact.

We will review and respond within 5 business days.

Good Faith Estimate

Under the No Surprises Act, you have the right to receive a Good Faith Estimate of the expected cost of services before your appointment.

If you are uninsured or choosing to self-pay, contact us to request your Good Faith Estimate prior to your first appointment.

If your actual bill exceeds the Good Faith Estimate by $400 or more, you may dispute the bill.

Electronic Agreement

Reading this webpage alone is not acceptance of this agreement. When acceptance is required, we will request an affirmative checkbox or electronic signature in the applicable intake or booking flow before proceeding.

Electronic records and signatures may be used to the extent permitted by applicable law.
