Informed Consent, Liability Waiver, and Telehealth Services Agreement

ONE Nutrition provides medical nutrition therapy and nutrition counseling services delivered by licensed Registered Dietitian Nutritionists. Services are provided via secure HIPAA-compliant telehealth platforms.

Nutrition services are not a substitute for emergency medical care or primary physician care.

Your participation is entirely voluntary. You may withdraw from services at any time without penalty.

You have the right to ask questions about your care, to receive a copy of your records, and to be treated with dignity and respect.

While nutrition counseling is generally safe and beneficial, results are not guaranteed and vary based on individual factors including adherence, health history, and medical conditions.

Recommendations provided are based on the information you disclose. Incomplete or inaccurate information may affect the quality of care.

If you experience a medical emergency, you must contact emergency services by calling 911 immediately. Do not rely on this service for emergencies.

Sessions are conducted via video or phone using HIPAA-compliant technology.

Telehealth has limitations compared to in-person care. Your dietitian may refer you to in-person services when clinically appropriate.

You are responsible for being in a private, secure location during sessions.

Technical failures may interrupt sessions. Your dietitian will make reasonable efforts to continue care.

Telehealth services may not be appropriate for all conditions. You acknowledge this has been discussed with you.

All health information is protected under the Health Insurance Portability and Accountability Act. We use HIPAA-compliant platforms for all communications, records, and billing.

To the maximum extent permitted by law, you agree to hold harmless and release Optimal Nutrition Experts, LLC, its owners, employees, dietitians, and contractors from any claims, damages, or liabilities arising from nutrition counseling services, except where caused by gross negligence or willful misconduct.

You acknowledge that you are responsible for your own health decisions and that recommendations are advisory in nature.

If you are booking on behalf of a minor under 18, you confirm you are the parent or legal guardian and have the authority to consent to services on their behalf.

By checking the confirmation box during booking, you confirm that you have read, understood, and agree to the terms of this Informed Consent, Liability Waiver, and Telehealth Services Agreement.

This electronic agreement is legally binding under the Electronic Signatures in Global and National Commerce Act.
